Voluntary Covid-19 self-report form for students
What would we like you to report?
Please only report a suspected or confirmed case of infection if you are the affected person. Do not use this form to report someone else. Please inform the university promptly!
What is a suspected or confirmed case?
Suspected case
According to the Robert Koch Institute (RKI), a Covid-19 should be suspected if
- you have one or more Covid-19 symptoms
- AND you have been in contact with someone who has tested positive for Covid-19
Being 'in contact', specifically, means that you shared the same space (e.g. in a lecture theatre, seminar room, library, computer room or any other room on or off-campus) at one point within 14 days before you developed symptoms, while your contact person was symptomatic.
Please do not report cases of the common cold, but only report if you suspect a Covid-19 infection based on contact with a confirmed case.
You should also visit a doctor to get a proper diagnosis. However, before you do, call your doctor on the phone and discuss with them how to proceed.
If you are tested and the result comes back negative, please use the online form below to let us know that you are no longer suspected of having Covid-19.
Covid-19 infection
Please report your Covid-19 infection to us directly. Although there is an official reporting chain, depending on how the infection rates develop, there may be a delay before the university is informed by the health authorities. If you inform us directly, this would allow us to take appropriate measures more quickly than would otherwise be the case.
